The three kinds of antifungals that have traditionally been in use are polyenes, azoles, and echinocandins.

But each one of them also comes with its own set of drawbacks. Polyenes, the oldest of the three main antifungal drug classes, target fungal cholesterol. The most commonly used member of this class is amphotericin B. While it radically improved options for patients when it was approved in the late s, it is associated with serious and sometimes life-threatening side effects, including fever, inflammation of the heart, and kidney problems.

As a result, these drugs have limited applicability. The first azoles were approved in the early s. They have been the most successful antifungal to date, mostly because they are available in an oral formulation. But they also come with problems. These drugs inhibit cytochrome p — a liver enzyme needed to detoxify blood.

As this enzyme metabolizes many drugs, interactions between azoles and other drugs can contribute to adverse events such as liver toxicity. Echinocandins target glucan synthase, an enzyme in the cell wall of fungi that is unique to fungi.

This makes them stand out as it means they have low levels of toxicity compared to the other two drug classes. When you consider that some patients might need treatment for months, this can be very challenging. Antifungal resistance has been creeping up for some time.

Bacteria mutate more readily and spread those mutations more readily than fungi. Resistance to antifungals in the azole class has become a particular problem, due largely to excessive use of azoles in agriculture. Flowers and fruits are particularly susceptible to fungal infections and low concentrations of azoles have been sprayed on fields in Western countries for many years.

A notable example of this is in the Netherlands, which has many tulip and flower farms. Aspergillus is a very common fungus that primarily lives on plants but can infect and severely damage the lungs of immune-compromised people.

Intensive spraying of the flower fields with low concentration azoles has led to the Netherlands having some of the highest rates of azole-resistant aspergillosis infections in the world. Recognizing that antifungal resistance was becoming a problem, regulations came in between and in both Europe and the US recommending that azoles should no longer be used as first-line therapy for fungal infections, such as aspergillosis or invasive candidemia.

However, Stein believes many healthcare practitioners have continued to prescribe azoles to their patients despite this advice, because of the convenience of the oral formulation.

That drives resistance. Innovation has been fairly slow in the antifungal space since the first echinocandins were approved 20 years ago. However, several biotech companies have been working hard to get new treatment options onto the market. In June , the FDA approved the first antifungal belonging to a brand new class in over 20 years.

Known as ibrexafungerp, this broad-spectrum antifungal belonging to the fungerp class has been developed by the biotech firm Scynexis based in New Jersey, US.

The drug is approved for the treatment of vulvovaginal candidiasis and is being tested for treating aspergillosis and other invasive fungal infections. There are another three new antifungals being tested in phase II and phase III clinical trials: rezafungin from California-based firm Cidara Therapeutics, olorofim from UK biotech F2G, and fosmanogepix from New York-based Amplyx Pharmaceuticals — which was acquired by Pfizer in April So anything else is a bit far off in terms of marketing authorization.

Currently being tested in phase II trials, the drug targets a broad range of different fungal infections.

When the capsule form is used, acidic drinks eg, cola, acidic fruit juices or foods especially high-fat foods improve absorption of itraconazole from the GI tract. However, absorption may be reduced if itraconazole is taken with prescription or over-the-counter medications used to lower gastric acidity.

Several medications, including rifampin , rifabutin , didanosine , phenytoin , and carbamazepine , may decrease serum itraconazole levels. Itraconazole also inhibits metabolic degradation of other medications, elevating blood levels with potentially serious consequences.

Serious, even fatal cardiac arrhythmias may occur if itraconazole is used with cisapride not available in the United States or some antihistamines eg, terfenadine, astemizole, perhaps loratadine. Rhabdomyolysis has been associated with itraconazole -induced elevations in blood levels of cyclosporine or statins.

Itraconazole may increase the serum concentration of certain medications eg, tacrolimus , warfarin , digoxin and therapeutic drug monitoring is recommended when these medications are used with itraconazole. A new formulation of itraconazole SUBA- itraconazole , for SUper BioAvailable has improved bioavailability without the need for an acidic environment in the stomach.

SUBA- itraconazole is taken with food and can be used to treat histoplasmosis, blastomycosis, and aspergillosis. Its dosage is different from other forms of itraconazole.

This broad-spectrum triazole is available as a tablet and an IV formulation. It is considered the treatment of choice for Aspergillus infections aspergillosis Aspergillosis Aspergillosis is an opportunistic infection that usually affects the lower respiratory tract and is caused by inhaling spores of the filamentous fungus Aspergillus , commonly present in read more in immunocompetent and immunocompromised hosts.

Symptoms of rhinocerebral Posaconazole can also be used as antifungal prophylaxis in patients with neutropenia with hematologic malignancies and in bone marrow transplant recipients. Adverse effects of posaconazole , as for other triazoles, include a prolonged QT interval and hepatitis.

Drug interactions occur with many medications, including rifabutin , rifampin , statins, and various immunosuppressants. They are available only in an IV formulation. Their mechanism of action is unique among antifungals; echinocandins target the fungal cell wall, making them attractive because they lack cross-resistance with other drugs and their target is fungal and has no mammalian counterpart. Drug levels in urine and CSF are not significant.

Echinocandins available in the United States are anidulafungin , caspofungin micafungin , and rezafungin. There is little evidence to suggest that one is better than the other, but anidulafungin appears to interact with fewer medications than the others.
